I agreed for Manaka's pull-up confession over Nishino. It was original, and everyone will remember this scene forever.
But I think the most emotional and dramatic confession was in Kimi ga Nozomu Eien, a confession Mitsuki Hayase did to Takayuki after Haruka Suzumiya's parents told him not to seeing Haruka in hospital anymore.
The background scoring was superb and made this scene more dramatic, in the end it took me to the deepest sympathy over Hayase and Takayuki's fate.

i think it would be Ayase and seimura from midori no hibi ( the manga )... The confession was nothing special, but after that, the way ayase ran cheerfully and suddenly stop under a street light and cries her heart out really touches my heart... I fell like crying too -_-"

The most amsuing and painful confession I saw were probably the separte confessions of Keitaro and Naru from Love Hina. He confesses, she runs away, and then she confesses 3 volumes later only after they fall off a cliff. I must admit, I cried at that point.
